I have a very similar hand size to yours, and I have tried many mice in the past few weeks. I usually play with a relaxed claw grip. If any shop where you live offers a return policy or allows you to try mice in their shop, I would suggest that. Your palm should have decent support, and all of your fingers should be able to rest naturally. I didn't like the Viper V3 Pro because it was too narrow in the middle, and the clicks caused my fingers to go into a V shape. My pinky had no place to rest. The Maya X and Superlight 2 are much more comfortable and universal for symmetrical mice. For ergos, I might recommend the EC2 or Xlite V4 medium, which are very popular shapes. I believe that mice under 122x62x40 will feel weird if you sometimes palm your mice and are used to good palm support. Smaller mice would work, but you need a different grip style with your hand size.
xlite v4 or es version would be great choice. u can also consider ec2dw and zywoo chosen.
there is some difference between ec2dw and xlite4 on the shape. i cant say which one is better. you can check some reviews on youtube about the comparision between these mice. vaxee e1 can also be included into the list.

The V4 feels like a letdown. I have minor switch grinding on LMB/RMB, Flexing on the sides that actuate the side buttons and rough edges where they glued the plastic base together with the top.
Nice write up. What's your hand size? Also use this to check dpi https://mousedpianalyzer.com/ put in the length of your pad and then do end to end measurements and you can test if one is way out. I've had some seriously big deviation over the years from 350-430 between mice. I'm waiting to pick up the ec2. Currently on the xlite v4 size 2
Vertical mice help the best for the pain I believe. Razer seem to have released Click V2 vertical so that's an gaming oriented option. Another option is to use very light asymmetrical "ergo" mouse. Most obvious recommendation is deathadder v3 pro or v4. For vertical mouse you don't need to consider weight. I've also suffered pain in the same areas and still do a little. I did do strecthes which increased the pain at first but alleviated it eventually long term and started using lighter mouse with lighter scroll wheel button (I suspect very stiff middle mouse button added pain). I also recommend a healthy dpi range (800-1600). Too little or too much and you are going to use too much of your wrist or shoulder. Right now I use deathadder v3 pro or Pulsar v4 medium. Good luck! So i have the same hand size as you, iv use several mice rn. Lamzu thorn Lamzu maya Razer basilisk Razer deathader v3 Logitech superlight Pulsar x2h size 2 Pulsar xlite v4 size 2 And rn im ricking susanto-x They are all different and tbh i thought i had to stick to ergos for gaming, and i loved my xlite v4 size 2, it was perfect fit. But a little too bulky or dense ar the time of high presure, small adjustments. So I finally tried the susanto-x. Boy o boy pumsar has outdone themselves this time. Like i get vaxee and lamzu, but vaxee still too heavy and a lil bulky imo and mechanical switches sound good but tbh too loud and longevity is not the best in mechanical, they can develop post and pre travel way faster. Battery life and QC in the susanto-x is better than logitech, razer and lamzu by far! And if you want to feel something fresh and very top notch eaither susanto-x or wait for new crazylight line maybe lare Q4 early Q1 2026.
If you used superlight 2 and you liked that shape, pulsar just released a new shape colab, pulsar susanto x (it is similar but not same exact shape). A lot of people said is tood, you can watch some reviews on youtube. I have xlite v4 size 2, lamzu thorn, razer basilisk, and pulsar x2h. Now I am waiting in my new susanto X.
I have kind of your same hand size, have thorn, and is too small. Zywoo is kind of a little bigger not much. Go with xlite from pulsar size 2, i had it and is very nice, if not size 3 at most. Smaller than those 2 mice and it will be too small for your hand. Check maybe vaxee np ergo idk how big it is but the new one is regarded as one of the best mice this year and overall. Rn im rocking maya X 10/10!!
If you have not try them out, I recomend a lot pulsar, specifically for you size just like me. I have tryed the X2H V3 size 2 for claw (i realized I do not like claw) Then I bought xlite v4 size 2, best ergo imo for my hand size (same as yours) I do relaxed palm with mix of claw sometimes. Perfect! Iv had razer viper v2 mini, razer basilisk, lamzu maya, lamzu thorn. Pulsar just clicks for me.
21x12cm hand You get used to lighter mice tbh I have an Xlite v4 now and it was significantly larger than my previous Viper and Superlight, but as long as you can get a comfortable/good grip it should be all good. I think pricing and quality control are the only bothersome things about gaming mice rn, there are so many shapes and sizes now that you'll be able to find something that works for you.
I think the Xlite v4 is around 60g? I think around 50g or lower would probably be too light for me on a large mouse. This mouse already feels like you're not holding anything, I can't imagine going lighter would feel great. I think shape is far more important than weight (within reason)
I went from a DAV3 to a xlite v4 and im very happy. Weighs less, good sensor and slightly narrower build is perfect for my claw pincir grip
I have pulsar xlite v4 med. Before i had gpx 2 dex,superlight 2,viper v3 pro,vaxee outset ax. For me xlite v4 is the best mouse if you like ergo shape
