The Naga Hex in Wraith Red. The moment I bought my first one I wished I'd had enough cash to buy a 2nd because suddenly you couldn't find them anymore and it was honestly just a beautiful mouse to look at.
I would break your question on 2 sections and view them separately: 1) Ergonomics. Don't take anyone's advice on this, you have to figure it out on you own. The best way is to go to some nearby shop that has a test mouses set up, so you could sit and just feel how they sit in your hand, then remember the shape of the most comfortable one and narrow your choices to that shape/size. All of the features will be meaningless if you'll not be able to comfortably use your mouse for a prolonged time. 2) Side buttons. Your main question. They are indeed very useful, especially for some classes with many slot-only skills, but I would not go with 12+ side buttons MMO mouses, they're more for slow-paced piano-style MMOs (like WoW, La2, etc.), in fast-paced games you have to have a fast response, precision and a good grip (including your thumb), basically what you're looking for is called MOBA mouse, but unfortunately they aren't very popular and there aren't many of those available. The best example of well-designed MOBA mouse is a Naga Hex, but those are not longer produced, there are other styles like few on top few on bottom, all on front/top/bottom, etc. from most of the well known brands and no-name Chinese ones, just try to find the one that has a thumb-rest positioned where you'll feel comfortable + Ergonomics and go with that. In worst case scenario get the FPS ones with 2-3 side buttons, still better than no buttons or piano on the side. MOBA mouses are also quite useful for work on your computer, as they don't interfere with the grip and give you extra options to boost the speed of your work.
I used a Naga Hex version 1 for almost a decade. I loved that mouse. It finally died on me and the v2 did not feel the same at all. I could only find refurbished v1s for like $200. I rarely used the side buttons but man I loved the feel of that mouse.
This is nonsense, mouseover healing and click healing are already baked in native into the game. Nothing is going to change there. Nor is side button support (which are just mapped keybinds) going away. I used a Razer Naga Hex V1 for many years on a Mac and it worked perfectly fine. OP just seems to have a software issue with their current mouse. I had a Naga Hex for like 12 years and it NEVER gave me issues. Such an amazing beginner gaming mouse. Only 6 buttons on the thumb and they were spread enough to make it REALLY easy to build the muscle memory. My cat decided the cord was a chew toy or I'd still have it. So naturally I bought a Naga Trinity because I had such a good experience with the Hex. Honestly, the Trinity was dog shit by comparison. Not even 6 months later I was having inconsistent issues with the buttons. Sometimes they would double trigger and sometimes they wouldn't at all. Not even a year later I switched to the Logitech G600 and it was kind of annoying to get set up, but it has been great so far. I've had it for over a year and have had no issues. I wasn't sure if I could get used to that many buttons on my thumb, but I did fairly quickly.

I miss the original hex so much. I have a Trinity and the scroll wheel jumps all over it sucks so much
The Razer Naga line is MASSIVELY under-appreciated. I'm on my third variant of the Naga. First was a red Naga Hex V1, second was a Hex V2, and current is a Naga X. All three still work, even the Hex V1, which I've had for around 14 years. Super comfortable, and really durable.
most of the people that have tried my mouse have complained about how big it is, so might do ok for you. Razer Naga X, or really any of the mice from the Naga line are decently sized. I've had a Hex V1, a Hex V2, and an X, and all were similar size.
I've got decent sized hands myself, and I've had a lot of friends complain that the mice I use are too big. I've used a few different models from Razer's Naga line, namely the Naga Hex V1, Naga Hex V2, and Naga X.
I'm at like 5 years on my third Naga, still have my first two Nagas, as well, and both still work. My first was a red Naga Hex, and that thing is about 13 years old.

2012 era naga was the best they could charge me 200+ for a re edition of this and I would gladly pay
