I wear the same shoes. For road I wear 860 10.5 (US sizing) 4E. I have collapsed arches and was okay with the 860s as is, but am much happier with custom orthodics in them. For trail and winter running (the 860s are downright unsafe on ice) I wear new balance nitrel size 11 4e with custom orthodics. Before I got my orthodics I used high arch third party insoles (Sole Thin Wide - which sounds ridiculous but they are a thin padding / vertical height with a wide toe bed). The nitrels aren't sexy, and no one stocks them (I buy mine from Amazon), but they work. I have tried various "wide toe box" trail shoes and in the rare case where the toe box is wide enough, the mid foot is still too narrow.
