I use the powerbeats3, with a corded connection between the two so I don't drop them. The larger tips block out most of the noise. I work alone, so I'm not to worried about someone else being deaf to events around me.

I just bought the Powerbeats3. I have really small air canals and I freaking love these things. They don't hurt my ear. They stay in even when I go to sleep with them.

I tend to lean on ones with loops that go over the ear because my ear holes are small, too. I struggle with buds because they tend to fall out. Last gen Powerbeats work pretty good although they’re noise isolating not noise cancelling, but I hear the current gen is noise cancelling. Epic Air Sport by JLab is pretty good and are noise cancelling I have QC ultras but don’t like the bud format. You may want to look at the new Bose Ultra Open Earbuds. I don’t own them and they’re not noise cancelling but they’re more like an ear cuff and obviate the conversation about ear opening size

i'm on my 3rd pair of Powerbeats. Each pair has lasted me 3 years, at which point one of them starts to fail w/ a rapidly diminishing battery. But for me they're the most comfortable, have the best physical controls, sound pretty good w/ music, and they're easy to switch between devices. Downside is the cost. But i've had no issues w/ sweat, i wear them in the shower.

Ditto, I’ve had my PowerBeats since they came out. Very secure, sound quality is good enough for running. For training I run with both in. For Ultras I run with one in ear + one in pocket, and swap when the battery goes flat. For 10+ hour events I will carry the case and charge on the go, or just swap to another pair of headphones at the aid-station with bag drop. Siri integration is great for pause/skip commands, plus I can listen and reply to Messages on the go too, obviously only valid for iPhone users. Heart rate function on the newest PowerBeats is a bit of a waste of time for most ultra runners, as you’ll have a wrist or chest HRM. So just turn off the PowerBeats HRM and increase battery life. Case of the newest is smaller - so that’s a plus.

look if your ear really fits usual headphones. my ear looks different I can't "hook" headphones in, so they will fall out. I tend to use shokz, power beats or simple JBL charge with thingies over the ear.  I tried clip style, but I thought they are uncomfortable.

Before reading your description, my first thought was the Powerbeats, which I think probably are the best fit for what you’re looking for. Otherwise, I’ve seen a lot of brands offer a “sport” earbud, like JBL, Bose, Soundcore. If you’re open to “open” earbuds, Nothing and Bose also have options, but you won’t be getting ANC with those.
